Once we have received this Health History/Registration form and payment, your spot has been successfully
reserved.  Voluntary evaluations which involves weight/body fat % and measurements are held the weekend prior to
the start date and is included in the price of your Boot Camp - An email will go out a week prior to the new
month to schedule a appointment.  This form must be submitted PRIOR to participation in Boot Camp.
